Health Forms

Submit Medical & Contact Forms Mandatory

Please read through the following links carefully and be sure to submit ALL required documentation as early as possible. All forms are due by August 1st for Fall and January 1st for the spring semesters.

Health Insurance is required.

Health insurance is mandatory for all international students and they are required to show proof of personal health insurance prior to moving into the residence halls.

For questions about immunization requirements, insurance information, or any other health and wellness questions, please visit our Health and Wellness page or contact the Office of Student Life by phone at 716-926-8793 or by e-mail.

Students will not be allowed to move in to their residence hall until they have submitted all required health forms.

COVID-19 Requirements Mandatory

Following CDC and New York State Department of Health guidelines, our requirements for COVID-19 are subject to change. You can view our updated policies on our COVID updates page. All on-campus community members are required to be vaccinated. Medical and religious exemption approvals will occur within 48 hours of submission.

Hilbert College makes several services and resources available to support the educational process of all students. From academic services and help in the classroom to the counseling and career development, Hilbert College is committed to serving the needs of all students.

Completing the following forms allow us to custom tailor our services to your needs and ensure that you receive the best educational outcomes!

Degree-Seeking Students

Students that submit their enrollment deposit early will receive priority registration.

New students that have accepted their admissions offer by depositing will be contacted to schedule their registration appointment, based on the date of their deposit.

Fall Entry Term:  Deposited students will be contacted as early as March 31st to schedule their new student advisement appointment.

Spring Entry Term: Deposited students will be contacted as early as November 15th to schedule their new student advisement appointment.

Exchange Students

One of the key aspects of studying abroad is curating your schedule. This involves selecting the courses you will be taking in a foreign country and ensuring that they align with your academic goals and requirements.
